Under the general supervision of the Clinical Supervisor of the Roving Team, the Clinical Social Worker I/II (CSW I/II) works doing in person outreach in the single room occupancy hotels (SROs) in San Francisco's Tenderloin, South of Market, and Mission neighborhoods. The permanent supportive housing sites where you will be doing outreach house formerly homeless adults who have been placed in housing through a subsidy from the city. The candidate will seek to engage high risk mentally ill, medically complex and/or substance abusing clients to increase housing retention and lessen the risk of eviction and a return to homelessness through building relationship and linking clients to needed supports.
The individual will hold a caseload of clients for whom they will be providing community based clinical case management which includes; engagement, assessment, treatment planning, psychotherapy, crisis intervention, 5150 assessments and holds, substance use treatment, psycho education, referrals to community supports, coordination and collaboration with property management and support services staff at the housing sites, advocacy for services, communication with families, and mandated reporting as needed. They will coordinate and consult closely with acute hospitals settings and outpatient medical providers to increase the continuity of care for their clients. They will track and document their interventions, assessments and referrals in a timely manner using an electronic medical record (Epic). The candidate will also perform other duties as assigned. The CSW I/II should possess a basic understanding of the DSM V mental health diagnostic criteria, Harm Reduction principles, Trauma Informed Care and Anti-Racism. Working with diverse and multicultural teams, clients, and community, the incumbent will participate and contribute to culturally focused clinical interventions and will practice a commitment to cultural humility and anti-racism.
The Supportive Housing Teams have a two hour weekly clinical team meeting where we foster a supportive learning and working environment by engaging in clinical trainings, sharing community resources, peer support, and presenting clients to the team for feedback and support. There is also an hour of clinical supervision weekly to each clinician.
The job will involve the ability to lift 50 lbs, walking several miles a day and climbing stairs.
