Overview

Since 1937, Alton Memorial Hospital has cared for residents in Alton, Illinois, and the surrounding communities in a five-county area. Starting as a land gift to the community from the Smith family, Alton Memorial today is a full-service acute care hospital. Alton Memorial offers patients a variety of inpatient and outpatient services, including surgery services, medical imaging, interventional and diagnostic heart services, cancer care, rehabilitation, 24-hour emergency care, ambulance services and more.

Case Management, Utilization Review, Social Services: Provides admission and continued stay reviews on all patients for the purpose of compliance and the proper utilization of resources. Provides social service resources to patient's and their families.

Preferred Qualifications

Role Purpose

Provides clinical social work services to address the psychosocial needs of patients and families. Provides individual and group counseling, linkage to community resources, expertise in cultural diversity and advocacy. Collaborates with the health care team and ensures timely, cost-effective discharge and transition planning.

Responsibilities

Conducts screening and assessment activities to meet the needs of the discharged patient.Applies assessment findings to prepare a patient-centered plan of care.Applies and complies with all regulations and utilizes resources available to patients to manage their continuum of care.
