In collaboration with care providers and physicians, responsible for problem solving, prioritizing and facilitating patient care objectives for the assigned area.

Required Experience

Required:

• California Licensed Clinical Social Worker eligible
• Bachelor’s Degree
• BLS from the American Heart Association
• LA City Fire Card within 6 months of employment (PHGSH only)
• 1-Year health care experience

 Preferred:

• Master of Social Work
