    Description

    Clinical Social Worker – Hospital

    Location: The Woodlands, TX
    Contract: 17 weeks
    Schedule: Monday–Friday | 7:30 AM–4:00 PM
    Shift: Days

    Job Summary

    Provides comprehensive psychosocial assessment, care planning, and support to patients and families. Collaborates with interdisciplinary teams to address social, emotional, behavioral, and environmental factors impacting patient care and discharge planning.

    Key Responsibilities

    • Conduct thorough psychosocial assessments and develop individualized plans of care.
    • Evaluate patient and family psychosocial risk factors and reassess care plans as needed.
    • Collaborate with patients, families, and interdisciplinary teams to support family-centered care.
    • Develop action plans addressing social, emotional, physical, and environmental barriers.
    • Assist patients and families with end-of-life decision-making and provide grief and bereavement support.
    • Address complex family dynamics, barriers to care, and discharge planning needs.
    • Intervene and de-escalate challenging or crisis situations appropriately.
    • Support families involved in legally complex cases and connect them with appropriate community resources.
    • Coordinate community resources and services to support patient and family needs.
    • Maintain patient privacy and confidentiality of protected health information.
    • Document assessments, interventions, recommendations, and outcomes accurately in the medical record.

    Required Qualifications

    • Master’s Degree in Social Work (MSW)
    • One of the following active licenses:
      • LMSW – Licensed Master Social Worker
      • LCSW – Licensed Clinical Social Worker
      • LMSW-AP – Licensed Master Social Worker – Advanced Practice
    • Strong clinical assessment, critical-thinking, and problem-solving skills.
    • Excellent professional communication and listening skills.

    Preferred Experience

    • Hospital experience
    • Behavioral Health experience
    • Crisis intervention and complex psychosocial case management
    • End-of-life, grief, and bereavement support
    • Community resource coordination
