We are currently accepting applications for Clinical Specialists to join our CARE, FACT, and IMPACT teams.

These teams provide intensive, community-based support to individuals living with mental illness and other complex behavioral health needs. As a Clinical Specialist, you will work directly with clients to coordinate treatment, recovery, and support services while helping individuals build the skills and connections needed to live as independently and successfully as possible.

About the Position

As a Clinical Specialist, you will be an important part of a collaborative, multidisciplinary team. Your work will extend beyond an office setting and into the community, allowing you to build meaningful relationships with the individuals you serve and support them in real-life situations.

You will work alongside clients to identify their strengths, needs, and goals while helping coordinate the medical, psychiatric, social, vocational, and community resources that support their recovery.

What You'll Do

Responsibilities may include:

• Participating in the ongoing assessment and therapeutic engagement of clients
• Working with clients and team members to develop and implement person-centered, individualized treatment plans
• Coordinating assessments and evaluations to support eligibility and access to services
• Monitoring client progress toward identified treatment, recovery, and wellness goals
• Helping coordinate medical, social, psychiatric, co-occurring, and other community-based services
• Providing direct support, advocacy, and referrals based on individual client needs
• Supporting clients in accessing financial resources, vocational opportunities, employment, and training
• Assisting individuals with symptom monitoring and supporting their response to treatment and education
• Teaching and supporting daily living skills such as grocery shopping, hygiene, self-care, maintaining housing, and utilizing transportation
• Supporting clients in exploring employment and other opportunities that promote independence
• Instructing and supporting clients in the self-administration of prescription and nonprescription medications, as appropriate
• Transporting and accompanying clients to appointments and community-based services as needed
• Advocating for clients to access needed services, benefits, and financial entitlements
• Helping connect clients and families with self-help groups, support systems, and other community resources
• Engaging family members and other natural supports, with appropriate consent, to provide education and connection to resources
• Participating actively in team meetings and supporting an integrated, multidisciplinary approach to care
• Maintaining accurate and timely documentation in accordance with agency, regulatory, and HIPAA requirements
• Supporting a positive, respectful, and collaborative work environment

Education & Experience

Candidates must have a minimum of a High School Diploma or equivalent.

Preferred qualifications include:

• Experience working with individuals in a case management, behavioral health, healthcare, or human services role
• An Associate's or Bachelor's degree in a human services-related field
• Experience working directly with individuals experiencing mental illness or other behavioral health challenges

Because this position involves providing services in the community, candidates must be comfortable traveling throughout the service area and transporting clients as needed.
