Review and attest to the applicant’s need for psychosocial rehabilitation services and medical and supportive activities to address the desired recovery goals.
Assure that a statement authorizing the proposed psychosocial rehabilitation services under the standards set forth is provided and filed in the consumer service record.
Provide feedback on the 30-dayinitialassessment and summary.
Review and sign all assessments and recovery plans.
MHP will be available to the service facilitators for consultation, staffing, and team/family meetings in the office Monday-Friday, 8:00 AM to 4:30 PM.
Attend andparticipatein CCS interdisciplinary recovery team meetings.
Develop, implement, and evaluate service plans with recovery team if assigned in a service facilitator role which alsoincludesevaluating the current DSM V diagnosis.
Provide clinical treatment psychotherapy to CCS consumers on an individual,groupand family basis.
Function as CCS Service Facilitator as assigned by CCSSupervisorand coordinate resources to meet specific consumer needs.
Consult with community agencies and families tomaintaincoordination and involvement in treatment process.
Coordinate consumer’s crisis services but not actually provide crisis services. Crisis services are provided by DHS 34 certified program.
Maintain confidentiality of client-related information,maintainrespectful treatment of consumers.
Provide courtroom testimony as needed.
Complete orientation training asand meet regularly for clinical supervision.
Maintain skills and competence levels as policies and regulations change, attend department/staff meetings/trainings,andparticipatein continuing education training as.
Adhere to approved social worker principles, methods, and practices, including the National Association of Social Worker’s (NASW) code of ethics.
