Summary:

• Provides facility-based therapeutic services to residents of the PJJSC, working in collaboration with the Behavioral Health Unit, including therapists, certified nurse practitioner and psychiatrist. These services are in close consultation with and supervision from the Program Manager. 

Responsibilities:

• 
  Provide residents as they are admitted to the Philadelphia Complete assessments, tools, and scales in the Philadelphia Juvenile Justice Services Center with residents according to our treatment model.
  
  Assess residents risk level and need for clinical services and work effectively with the treatment team to identify treatment barriers and needs.
  
  Maintain clinical records reflective of all clinical contact points with residents as indicated by Hall Mercer JJSC policies and procedures.
  
  Supports residents in navigating admission to and discharge planning from the PJJSC.
  
  Research and refer residents to resources and clinical services as needed considering stability and clinical outcomes.
  
  Participate in weekly rounds, supervision, training with the program manager, medical director, and other team members, as well as departmental and agency meetings.
  
  Participates in after-hours crisis consultation on a rotating basis between mental health staff.

Credentials:

• LSW or LCSW or LPC or LMFT (Preferred)

Education or Equivalent Experience:

• Master of Arts or Science 
• Social work, psychology, rehabilitation, activity therapies, counseling or education At least 1 year of experience as a direct care provider.
