    Description

    Basic Function:        Provides administrative and operational support to the State Home Care (SHC) Care Management and Nursing teams to promote efficient workflows, maintain regulatory compliance, and ensure timely coordination of consumer services.

     

    Responsibilities:

    1. Perform administrative and clerical functions in support of the State Home Care Program, ensuring compliance with the Executive Office of Aging and Independence (AGE) requirements and organizational policies.

     

    1. Maintain accurate electronic and paper consumer records, including data entry, document management, SharePoint records, Microsoft Office applications, and file attachments within WellSky.

     

    1. Update Activity & Referral (A&R) records and forward annual care plans to facilitate continuity of services and timely reassessments.

     

    1. Coordinate provider communications regarding schedule changes, ensuring timely notification of appropriate staff.

     

    1. Scan, upload and electronically file assessment related documentation into WellSky and other electronic record systems, ensuring records remain complete and current.

     

    1. Manage MassHealth ELIRA report; submit PSI and Frail Elder Waiver notifications to the Medicaid office.

     

    1. Coordinate consumer correspondence by preparing and distributing required documentation, educational resources, and notifications.

     

    1. Assist with the orientation and training of new department staff on departmental administrative procedures and workflows.

     

    1. Monitor Patient Ping/Bamboo Health notifications for consumer hospital admissions and discharges and notify Care Management staff for appropriate follow-up.

     

    1. Monitor and maintain monthly WellSky reports to support compliance with AGE requirements and internal quality standards.

     

    1. Provide reception and administrative coverage as needed to ensure continuity of office operations.
