General Summary: Responsible for providing routine daily nursing care and services in accordance with the resident’s assessment and care plan and as directed by your supervisor. This care is inclusive of but not limited to activities of daily living, assistance with therapeutic activities program, restorative programs, and more.
Essential Job Duties:
- Ensure resident rooms are ready for new admissions.
- Assist in completing the personal inventory sheet with the resident.
- Store resident’s belongings neatly.
- Attend and participate in nursing reports upon reporting for duty.
- Assists residents with dressing, grooming, eating, bathing, personal hygiene, positioning, turning, toileting, and exercising.
- Assists residents with daily dental and mouth care.
- Assist residents with bathing support.
- Assist residents with fingernail care. Report concerns to the nurse regarding all nail care.
- Assist female residents with maintaining a clean-shaven presence (i.e., facial hair, underarms, legs, etc.).
- Assist male residents with maintaining facial hair as desired.
- Turn and reposition residents frequently (at least every 2 hours).
- Assist resident with prompt incontinent care.
- Provide residents with indwelling catheter care.
- Assist resident with application of devices (i.e., splints, slings, binders, etc.)
- Assist resident with mobility devices.
- Provide an active and passive range of motion.
- Assist nurse with collection of specimens.
- Assist residents in preparing for medical tests.
- Assist residents in preparation for meals (i.e., clean hands, clean surfaces, cutting foods, etc.).
- Serve trays.
- Prepare and pass fresh ice water/alternate in accordance with physicians’ orders.
- Serve between meals and bedtime snacks.
- Assist resident with after-meal care (i.e., clean hands, face, clothing, brush teeth/dentures, etc.).
- Assist residents in preparing for activities and social programs.
- Assist with obtaining weights and vital signs promptly.
- Ensure resident call bell is always within reach.
- Reports and records observations and results of treatments, tests, procedures, and specimens when indicated.
- Record support and self-performance, intake, etc. provided to the resident each shift.
- Transfers, transports, and escorts residents as needed.
- Answers signal lights promptly.
- Changes bed linens, make beds, and keeps rooms clean and orderly.
- Assists residents in performing personal laundry as needed.
- Familiarity with emergency procedures and may be required to assist with evacuations.
- Reports all resident changes in condition promptly to the Nurse.
- Reports all allegations, accidents, and incidents to the Nurse.
- Maintain infection control practices.
- Attends in-service training and education sessions, as assigned; completes Relias training on time every month.
Other Duties:
- Maintains safe and sanitary workstations and equipment.
- Encourages independence, attendance at activities, restorative programs, and rehabilitation.
- Operate as a member of a team both within the department and across departments.
- Performs other work duties and responsibilities as assigned.
