WORK INDEPENDENTLY · 1-to-1 IN-HOME PATIENT CARE

RESPONSIBILITIES:
- Perform home visits for patients as delegated by the care managing RN
- Observes, records, and coordinates care with multi-disciplines regarding the patient's treatment and any change in condition
- Supports the patient and family as appropriate to the patient’s condition
QUALIFICATIONS:
- Current certification as a CNA in Connecticut
- Valid driver’s license
- Reliable transportation and willing to travel in New London County
- Strong communication and interpersonal skills
- Home health experience preferred
