Suncoast Hospice, a member of Empath Health, is seeking a caring and motivated CNA to provide personal care and tasks related to the activities of daily living for patients in their homes in North Pinellas County on the following schedule Monday - Friday from 8am to 5pm.

What youll Do

• The provision of hands-on personal care and assistance with activities of daily living.
• Assisting patients with taking medications that are ordinarily self-administered and have been pre-poured as designated in the Plan of Care.
• Hospice Aides must report changes in the patients medical, nursing, rehabilitative, and social needs to a registered nurse, as the changes relate to the plan of care. Hospice Aide must document the changes and to whom they were reported to.
• Following a patient's death collaborates with primary RN/Counselor and participates as needed in closure contact with family/caregivers.

What You'll Need

• Experience: One to three months related experience and/or training preferred.
• Valid Florida Board of Nursing certification (CNA License).
• Valid Florida State Driver's License
• For field-based positions, employees must have reliable transportation which will enable them to perform tasks and responsibilities in a timely and appropriate fashion. Must provide proof of valid automobile insurance, a copy of which will be placed in the employee's HR file.
