Responsibilities:<\/span><\/b> <\/span>
<\/div><\/div>
Under the direction of a Registered Nurse or Case Manager, will be required to: <\/span>
<\/div>
- Recording pertinent information pertaining to the patient’s care. <\/span><\/span>
<\/li> - Reminding patients to take medication at the appropriate time.<\/span><\/span>
<\/li> - Planning and preparing meals according to specific dietary requirements of the patient, and if
necessary, feeding the patient.<\/span>
<\/li> - Assisting the patient with therapy exercises under the direction of a Registered Therapist. <\/span>
<\/li> - Assisting or performing personal care including hair care, oral hygiene, bathing, grooming,
and dressing. <\/span>
<\/li> - Assisting with ambulation with or without mechanical aids. <\/span>
<\/li> - Assisting with routine bodily functions: Example, toileting. <\/span>
<\/li> - Maintaining a safe, clean and healthy environment through light housekeeping including
changing bed linens, dusting and vacuuming, cleaning kitchen and bathroom, and laundry.<\/span>
<\/li> - Providing companionship and stimulation for the patient including reading, walks, etc.; and
accompanying the patient to doctors or other appointments. <\/span>
<\/li> - Grocery shopping or other errands when needed. Performing other housekeeping tasks as indicated in the care plan. <\/span>
<\/li> - Taking patient’s vital signs and recording input and output. <\/span>
<\/li> - Observing and reporting changes in patient’s condition to the registered nurse. <\/span>
<\/li> - Accurately preparing daily records and submitting them to the office by date due. <\/span>
<\/li> - Participating in in -service education programs. Do not accept any form of financial support
from a patient or family members.<\/span>
<\/li><\/ul><\/div><\/span>
Requirements<\/h3>- Must have successfully completed a CNA training course; or have successfully passed an
equivalency exam and posses written evidence of such completion. <\/span><\/span>
<\/li> - Copy of CNA certification (at hire) <\/span><\/span>
<\/li> - Copy of current Tb/PPD test results (at hire and every 2 years) <\/span><\/span>
<\/li> - Copy of Hep -B Vaccination dates or declination statement (at hire) <\/span><\/span>
<\/li> - Copy of current BLS / ACLS certification(s) or other specialty certifications as
necessary (at hire and current) <\/span><\/span>
<\/li> - Completed Skills Competency Assessment (at hire) <\/span><\/span>
<\/li> - Copy of Annual Medical Examination (at hire and current)<\/span><\/span>
