Keystone Care is actively hiring CNAs and Home Health Aides (HHAs) to provide compassionate care for hospice patients in their homes, as well as in nursing homes and assisted living facilities throughout the Delaware County area. If you are reliable, dedicated, and ready to make a meaningful difference, we have immediate shifts available.

We are hiring urgently due to high patient demand — consistent hours available
What You’ll Do
Provide compassionate, hospice-focused care to patients with life-limiting illnesses in their homes, nursing homes, and assisted living facilities
Assist patients with activities of daily living, including bathing, dressing, grooming, and personal hygiene, with dignity and respect
Offer emotional support and companionship to patients and their families during end-of-life care
Assist with meal preparation and feeding, accommodating dietary needs and patient comfort
Perform light housekeeping to maintain a clean, safe, and comfortable environment
Support safe mobility, transfers, and repositioning to promote comfort and prevent skin breakdown
Travel between patient locations as assigned, demonstrating flexibility and reliability
Observe, document, and promptly report changes in patient condition, behavior, or comfort level to the care team
Provide comfort-focused care, including positioning, oral care, and other non-medical interventions aligned with hospice goals
Accompany patients to appointments or procedures when needed
Requirements
Related caregiving experience required; CNA or HHA experience preferred.
Prior hospice or end-of-life caregiving experience required.
Experience in assisted living facilities required.
Experience providing companionship care required.
Reliable transportation.
Ability to communicate effectively in English.
High school diploma or equivalent.
Ability to safely perform patient transfers, repositioning, and mobility assistance.
Active CNA certification or HHA credential.
Eligibility to work in the United States.
Availability to work day shifts of 6:00 AM–2:00 PM or 7:00 AM–3:00 PM.
