The Hospice Home Health Aide (HHA) provides personal care services to hospice patients in their homes, assisted living facilities, or nursing facilities under the supervision of a Registered Nurse. The ideal candidate is compassionate, dependable, and comfortable working independently while maintaining excellent communication with the hospice interdisciplinary team.
Responsibilities
Assist patients with activities of daily living (ADLs), including bathing, grooming, dressing, oral care, toileting, transfers, and ambulation
Provide routine skin care and assist with patient comfort measures
Observe and report changes in patient condition to supervising nurse
Assist with light housekeeping related to patient care
Maintain a safe, clean, and comfortable environment for patients
Document patient care accurately and timely
Follow infection control, safety, and hospice care protocols
Provide compassionate emotional support to patients and families
Communicate effectively with nurses and hospice team members regarding patient needs
Assist with office-related tasks and administrative support during non-patient care hours as assigned
Qualifications
Current CNA certification in the State of Texas
Hospice experience preferred
Current driver’s license, reliable transportation, and auto insurance
Ability to travel to patient homes and facilities
Strong communication and interpersonal skills
Compassionate, dependable, and professional demeanor
Ability to work independently and manage time effectively
CPR certification preferred
Schedule
Part-Time position averaging approximately 20 hours per week
Flexible scheduling based on patient census and agency needs
Employees may be required to complete office hours and assist with administrative duties to supplement hours not spent in direct patient care
Some weekends and holidays may be required
Physical Requirements
Ability to assist with transfers and repositioning of patients
Ability to stand, walk, bend, and lift during patient care activities
Ability to travel between patient locations throughout the day
