Job Summary:
Tryfacta is seeking a CNA – Long Term Care (LTC) for our client in Amsterdam, NY 12010. This is a 13-week contract assignment. If you meet the qualifications listed below and are interested, please Apply Now!

Job Description: Certified Nursing Assistant (CNA) – Long Term Care (LTC)
Location: Amsterdam, NY 12010
Duration: 13 Weeks
Available Shift: Day 6:00 AM to 2:30 PM, Evening 02:00 PM to 10:30 PM, Night 10:00 PM TO 6:30 AM
Guaranteed Hours: 40 Hours/Week (5x8 Schedule)

Key Responsibilities:
  • Assist residents with activities of daily living (ADLs)
  • Provide direct patient care and support in a Skilled Nursing Facility (SNF) setting
  • Monitor and report changes in resident condition to nursing staff
  • Assist with mobility, transfers, and personal hygiene
  • Maintain resident comfort, safety, and dignity
  • Document care provided according to facility policies
  • Collaborate with nurses and interdisciplinary care teams

Requirements:
  • Minimum 2 years of CNA experience required
  • Skilled Nursing Facility (SNF) and/or Long Term Care (LTC) experience required
  • Active New York State CNA License required
  • BLS Certification required
  • Must be able to work at least 4 out of 5 scheduled shifts per week
  • Must be available to work every other weekend
