Job Summary:
Tryfacta is seeking a CNA - LTC for our client in Clifton Springs, NY 14432. This is a 13-week contract assignment. If you meet the qualifications listed below and are interested, please Apply Now!

Position Title: CNA - LTC
Location: Clifton Springs, NY 14432
Duration: 13 weeks (Possible extension)
Work Schedule: 5x8-Hour (15:00 - 23:00) with EOW
Pay rate: 28-32$/hr.

Responsibilities for this position include, but are not limited to:

• Assist residents with activities of daily living (ADLs), including bathing, dressing, grooming, oral care, toileting, and feeding.
• Help residents with ambulation, transfers, repositioning, and mobility using transfer belts, wheelchairs, stretchers, and Hoyer lifts.
• Perform bed baths, shower assistance, linen changes, skin care, foot care, perineal care, and pressure injury prevention.
• Monitor and document intake/output, weight, drain output, urine collection, and changes in patient condition.
• Assist with specimen labeling and transportation as directed.
• Maintain a clean and safe patient environment by following infection control, PPE, and isolation protocols.
• Assist with oxygen equipment, specialty beds, and other resident care equipment.
• Report any changes in resident condition promptly to the licensed nursing staff.
• Collaborate with nurses and other healthcare professionals to provide quality resident care.
• Float to other units as needed.

• Minimum 1 year of Long-Term Care (LTC) experience required.
• Previous travel experience required (First-time travelers not accepted).
• Active BLS Certification required.
• Must live more than 50 miles from the facility (locals not accepted).
• Comfortable floating between units as needed.
• Ability to work every other weekend.
• Experience caring for adult and geriatric residents.
• Strong communication, teamwork, and patient care skills.

• Patient Ratio: Approximately 1:10
• Daily Census: Approximately 21 residents
• 23-bed Long-Term Care unit
• Experience with any electronic charting system is acceptable.
• COVID-19 vaccination is not required per current facility guidelines.
