Job Summary:
Tryfacta is seeking a CNA / Nurse Assistant - LTC for our client in Vestal, NY 13850. This is a 13-week contract assignment. If you meet the qualifications listed below and are interested, please Apply Now!

Position Title: CNA / Nurse Assistant -LTC
Location: Vestal, NY 13850
Duration: 16 weeks
Work Schedule: 8X5(Day, Eve, Night) | 3x12(Day and night) | 2x12(rotational)
Hours: 36/40hr.

Responsibilities for this position include, but are not limited to:
  • Provide direct patient care and assist residents with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, feeding, and mobility.
  • Assist residents with transfers, repositioning, ambulation, and the use of mobility devices while maintaining safety.
  • Obtain and document vital signs, weight, intake/output, and report any changes in resident condition to the nursing staff.
  • Maintain accurate and timely documentation of resident care provided in accordance with facility policies.
  • Support residents with Alzheimer's disease, dementia, and other long-term care needs while promoting dignity and independence.
  • Respond promptly to resident call lights and provide compassionate, patient-centered care.
  • Assist with meal setup, feeding assistance, hydration, and monitoring nutritional intake.
  • Maintain a clean, safe, and organized patient environment while following infection prevention and standard safety protocols.
  • Work collaboratively with RNs, LPNs, therapists, and interdisciplinary team members to ensure quality resident care.
  • Participate in orientation, required training, and complete Relias modules before assignment start.
  • Follow facility attendance policies, weekend scheduling requirements, and maintain professionalism at all times.
  • No medication administration (No Med Passes required).

  • Active New York State CNA License required at the time of submission ( No license hits allowed ).
  • Minimum 1 year of recent Long-Term Care (LTC) experience required. Candidates with recent Skilled Nursing (SNF), Rehab, Med-Surg, or Hospital experience will also be considered.
  • Current BLS Certification required.
  • Must provide proof of NY CNA license at the time of submission.
  • Experience caring for geriatric residents , including Alzheimer's and Dementia patients.
  • Strong knowledge of resident safety, infection control, and patient care best practices.
  • Excellent communication, teamwork, and interpersonal skills.
  • Ability to work weekends (every other weekend required) and maintain schedule flexibility.
  • PCC (PointClickCare) experience is preferred but not required.
