Job Summary:
Tryfacta is seeking a CNA / Nurse Assistant - LTC for our client in Vestal, NY 13850. This is a 16-week contract assignment. If you meet the qualifications listed below and are interested, please Apply Now!

Position Title: CNA / Nurse Assistant - LTC
Location: Vestal, NY 13850
Duration: 16 weeks
Work Schedule: Night 5x8-Hour (22:45 - 07:15)

**New York State License is required at submission with proof of license; no license hits allowed.
Shift cannot be changed once offered.
RTO must be submitted and approved at the time of submission; no additional RTO will be approved after offer acceptance.**


Responsibilities for this position include, but are not limited to:
  • Provide direct patient care to long-term care residents, ensuring safety, comfort, and quality of life.
  • Monitor, document, and report changes in patient condition to the healthcare team.
  • Assist residents with activities of daily living (ADLs), including mobility, hygiene, and feeding as needed.
  • Deliver compassionate care to residents with Alzheimer's disease and dementia while promoting dignity and independence.
  • Maintain accurate and timely documentation in the PointClickCare (PCC) electronic health record system.
  • Collaborate with nurses, physicians, therapists, and interdisciplinary team members to support resident care plans.
  • Adhere to facility policies, infection control protocols, and state regulatory requirements.

  • Active and unrestricted New York State License required at the time of submission.
  • Current BLS (Basic Life Support) Certification required.
  • Minimum 1 year of Long-Term Care (LTC) experience preferred.
  • Experience in rehabilitation, medical-surgical, hospital, or skilled nursing settings accepted.
  • Knowledge of caring for Alzheimer's and dementia patient populations.
  • Strong clinical assessment, communication, and documentation skills.
  • Ability to work independently in a fast-paced healthcare environment, including every other weekend and assigned shifts.
