Responsibilities

• Provide compassionate personal care to patients as a CNA on-site.

• Monitor and report changes in patient condition to supervisor promptly.

• Accurately document patient observations and care activities in charts.

• Collaborate with nursing and care teams to coordinate patient care plans.

• Demonstrate a growth mindset by seeking learning opportunities and skill advancement.

• Meet and exceed performance targets within assigned territory or area.

• Assist with daily living activities to support patient independence.

• Maintain a safe, clean, and organized care environment.

• Utilize care pipelines and workflows to forecast daily care needs and priorities.

• Provide empathetic, respectful communication with patients and families.

• Support on-site team with tasks to maximize efficiency and patient throughput.

• Maintain professional appearance and comply with facility policies and procedures.

Experience/Qualifications

• Certified Nursing Assistant license valid in the state of employment.

• Provide compassionate personal care and daily living assistance to patients onsite.

• Accurately document observations and care activities in patient charts.

• Ability to monitor patient condition changes and notify supervisor promptly.

• Collaborate with nursing teams to coordinate care plans and follow care pipelines.

• Display growth mindset with ongoing learning and skill advancement.

• Meet or exceed performance targets within assigned territory or area.
