Responsibilities
Provide compassionate personal care to patients as a CNA on-site.
Monitor and report changes in patient condition to supervisor promptly.
Accurately document patient observations and care activities in charts.
Collaborate with nursing and care teams to coordinate patient care plans.
Demonstrate a growth mindset by seeking learning opportunities and skill advancement.
Meet and exceed performance targets within assigned territory or area.
Assist with daily living activities to support patient independence.
Maintain a safe, clean, and organized care environment.
Utilize care pipelines and workflows to forecast daily care needs and priorities.
Provide empathetic, respectful communication with patients and families.
Support on-site team with tasks to maximize efficiency and patient throughput.
Maintain professional appearance and comply with facility policies and procedures.
Experience/Qualifications
Certified Nursing Assistant license valid in the state of employment.
Provide compassionate personal care and daily living assistance to patients onsite.
Accurately document observations and care activities in patient charts.
Ability to monitor patient condition changes and notify supervisor promptly.
Collaborate with nursing teams to coordinate care plans and follow care pipelines.
