Responsibilities
Deliver direct patient care under supervision with accuracy and empathy.
Document patient observations and care provided in electronic records daily.
Monitor and report changes in patient condition to team lead promptly.
Assist with activities of daily living while maintaining patient safety.
Adhere to policies, procedures, and regulatory requirements consistently.
Participate in ongoing learning and skill development aligned with growth goals.
Collaborate with the healthcare team to achieve measurable performance targets.
Demonstrate accountability for personal outcomes and contribute to team success.
Support care planning by collecting routine data and feedback.
Maintain cleanliness and safety of the patient care environment.
Utilize care pipelines and metrics to identify opportunities for territory or area improvement.
Assist with patient education and discharge planning when appropriate.
Experience/Qualifications
Active CNA license with current certification requirements fulfilled.

Accurate documentation of observations in electronic health records daily.
