Responsibilities

• Provide direct care and daily living assistance to patients as assigned.

• Monitor patient conditions and report changes to supervisors promptly.

• Engage in ongoing learning to advance clinical skills and certifications.

• Uphold patient confidentiality and adhere to regulatory and safety guidelines.

• Maintain accurate patient records, documenting treatments and outcomes.

• Collaborate with care team to develop and implement care plans.

• Support patients with mobility, housekeeping, and personal care needs.

• Operate and sanitize medical equipment per facility protocols.

• Assist in coordinating patient admissions, transfers, and discharges.

• Demonstrate strong time management to meet patient care targets and schedules.

• Participate in performance metrics and pipeline-driven planning for territory or function.

• Proactively identify opportunities for process improvements within care delivery.

Experience/Qualifications

• Certified Nursing Assistant (CNA) license valid in the state.

• Provide direct patient care and daily living assistance per care plan.

• Accurately document treatments and patient outcomes in records.

• Monitor patient conditions and report changes promptly to supervisors.

• Maintain patient confidentiality and comply with regulatory guidelines.

• Engage in ongoing learning and pursue certifications to advance clinical skills.

• Collaborate with care team to develop and execute care plans.

• Demonstrate strong time management to meet care targets and schedules.

• Operate and sanitize medical equipment following facility protocols.

• Assist with admissions, transfers, and discharges to ensure smooth transitions.

• Support mobility, housekeeping, and personal care as part of holistic care.

• Identify opportunities for process improvements in care delivery.

• Contribute to performance metrics and pipeline planning for care delivery.
