JOB PURPOSE:
The Collaborative Care Case Manager is responsible for carrying a caseload of dually diagnosed intellectual and developmental disabilities /behavioral health consumers who have multiple, complex needs and who would benefit from collaborative case management services to prevent potential crisis situations. The Collaborative Case Manager will provide consultation, training, and technical assistance for individuals with intellectual and developmental disabilities and the staff who support them in accordance with agency policies and operating procedures as well as applicable state and federal statutes and regulations. To provide consultation and supports for caregivers and community providers to ensure successful diversions from institutional placement and transition to community from institutional placement.
The best practice models developed in the Learning Collaborative and accompanying strategies aim to expand access to outpatient mental health services for individuals with IDD and mental health needs.? These services are designed for children and adults who have IDD or who are presumed to have IDD and who also have co-occurring mental health condition, substance use disorder, or behavior support needs.
