Job Summary
SummaryResponsible for providing direct support and supervision to individuals residing in community-based behavioral health or supportive living programs. The Community Residence Counselor assists residents in developing daily living skills, achieving treatment goals, and maintaining a safe and supportive environment. This role plays a vital part in promoting recovery, independence, and quality of life for individuals with mental health, substance use, or developmental conditions.Does this position require Patient Care?YesEssential Functions-Support residents with daily routines, medication reminders, meal preparation, transportation, and personal care as needed. -Monitor resident behavior and provide interventions consistent with treatment or service plans. -Document observations, progress notes, and incident reports accurately and in a timely manner. -Assist with the development and implementation of individualized service plans under clinical supervision. -Promote a respectful, inclusive, and safe living environment aligned with trauma-informed care principles. -Engage residents in therapeutic, recreational, and community integration activities. -Respond to crises using de-escalation and safety protocols and report to clinical leadership as appropriate. -Communicate effectively with care teams, family members, and external providers as needed. -Participate in team meetings, trainings, and quality assurance activities to support program effectiveness.Qualifications
