GENERAL STATEMENT OF DUTIES

Performs professional and consultative work in developing, implementing, and sustaining the Family Connects program in the Public Health Department.

DISTINGUISHING FEATURES OF THIS CLASS

Employees in this class manage, coordinate, and promote the development of community resources in partnership with both public and private agencies within the community. Work includes designing and implementing Family Connects marketing plans for the community. Employees are responsible for establishing, maintaining, and/or participating in effective coalitions and partnerships among civic, community and health and human services partners to plan programs, services, and supports designed to meet the needs of families with newborn children. The position requires independence in planning work, establishing schedules, and making program-related decisions. Work is performed under the general supervision of the Community Health Consultant.

Examples of Duties

DUTIES AND RESPONSIBILITIES

Essential duties and responsibilities include, but are not limited to:

• Establish, participate in, and nurture relationships, partnerships, and coalitions with community resources and services that provide support to families referred to by Family Connects nurses and that participate in the community system of care for young children and their families.
• Participates in Community Advisory Board(s) (CAB) to coordinate services with others in the early childhood system of care and collaborates to support Community Alignment across partners in Guilford County.
• Identify resources, establish relationships with providers, maintain an updated list of resources and identify gaps in needed community resources
• Attend nurse case conferences during team meetings to provide case consultation education about community resources to support nurse home visitors as they connect families with community resources
• Provide individual consultation about community resources to support nurse home-visitors as they connect families with community resources.
• Market the program by communicating with referral sources (ex: OB/GYN Practitioners, hospitals, pediatricians), communicating with groups that interact with families (for example employers, DSS, churches, childcare agencies), and soliciting support from community leaders (ex: to publicize the program and provide incentives to families if available).
• Compile and maintain reports, and documentation of program activities regarding community relations for use in program evaluation.
• Identifies gaps in supports for families using both quantitative and qualitative data and collaborates with partners, including Community Advisory Board(s), to elevate and address gaps and barriers to receiving services.

Typical Qualifications

MINIMUM QUALIFICATIONS

Bachelor's degree from an accredited college or university in the human services area or equivalent combination of education and experience that provides the necessary knowledge, skills and abilities to successfully perform the essential functions of the position.

PREFERRED QUALIFICATIONS

Master's degree from an accredited college or university in the human services area. Two or more years of demonstrated experience leading and participating in community initiatives for public health programming and/or project management.

This position will be out in the community and promoting the FCG program (meetings, health fairs, going to community & provider partner offices)

Driving Requirement

This position requires driving, whether driving a County owned or personal vehicle, to conduct county business. Driving includes, but is not limited to attending conferences, meetings, or any other country-related functions. Motor Vehicle Reports may be verified for valid driver's license and that the driving record is compatible with the county's driving criteria. If a personal vehicle is operated for county business proper insurance is maintained as per Guilford County's vehicle use policy.
