    Community-Based Hospice LPN (Field LPN)

    Full-Time | Monday-Friday | 8:00 a.m. to 5:00 p.m.

    What You'll Do

    Deliver Exceptional Hospice Care

    • Provide skilled nursing care to hospice patients in homes and residential settings under the direction of the Registered Nurse Case Manager (RNCM)
    • Assess patient needs and report changes in condition to the RN for evaluation and care plan adjustments
    • Deliver compassionate, patient-centered care in accordance with physician orders and the established plan of care
    • Support patients and families throughout the hospice journey with empathy, professionalism, and respect

    Support Patients and Families

    • Provide telephone support to patients, caregivers, physicians, and community partners
    • Help address patient concerns and coordinate care needs within the LPN scope of practice
    • Educate patients and families on hospice services, symptom management, medications, and end-of-life care
    • Ensure timely communication regarding changes in patient status

    Contribute to Clinical Excellence

    • Complete accurate and timely documentation
    • Assist with chart audits, supplemental orders, and other clinical support activities
    • Monitor and coordinate appropriate use of medications, supplies, and medical equipment
    • Participate in performance improvement initiatives and quality-focused activities

    Collaborate as Part of an Interdisciplinary Team

    • Work closely with nurses, physicians, social workers, chaplains, aides, and other team members
    • Participate in care planning and patient-centered decision-making
    • Support onboarding and training of new team members
    • Help ensure seamless, coordinated care for patients and families

    Required Qualifications

    • Current Licensed Practical Nurse (LPN) license in North Carolina
    • Minimum two years of recent clinical nursing experience
    • Current CPR certification
    • Reliable transportation
    • Strong clinical assessment and documentation skills
    • Competent computer skills
    • Ability to travel throughout the service area and provide care in patient homes and facilities
    • Ability to lift and/or carry up to 50 pounds
    • Ability to meet the communication, assessment, and documentation requirements of the role

    Preferred Qualifications

    • Hospice experience
    • Home health experience
    • Skilled nursing or long-term care experience
    • Hospice Nurse Certification
    • Knowledge of Medicare regulations, accreditation standards, and reimbursement requirements
