Core Responsibilities

  • Develop and maintain partnerships with local agencies, healthcare providers, government bodies, and community stakeholders
  • Monitor program budgets, track expenditures, and prepare financial reports to ensure fiscal responsibility
  • Collect and analyze program data to assess outcomes, identify trends, and recommend evidence-based improvements
  • Ensure full compliance with relevant federal, state, and local regulations, as well as internal policies and procedures
  • Lead community needs assessments and contribute to strategic planning processes
  • Respond to program crises and coordinate appropriate interventions in a timely and professional manner
  • Prepare and submit grant reports and assist with funding proposals as required
  • Facilitate regular team meetings, training sessions, and performance reviews

Must-Have Requirements
  • Bachelor's degree in social work, public health, healthcare administration, human services, or a closely related field; Master's degree preferred
  • Minimum of three years of experience in program management within a community care, social services, or healthcare setting
  • Demonstrated ability to lead and develop diverse, multidisciplinary teams
  • Strong knowledge of community resources, social determinants of health, and trauma-informed care principles
  • Proven experience managing program budgets and interpreting financial reports
  • Excellent written and verbal communication skills, including the ability to present to diverse audiences
  • Proficiency with case management software and standard office productivity tools
  • Valid driver's license and reliable personal transportation, as occasional travel between service locations may be required
