Community Case Manager - Adult Services - Brattleboro

The pay for this position is $44,850-$58,909 annually.

$500 sign-on bonus

The Community Case Manager works directly with individuals receiving services to build functional skills, address barriers to care and social and mental wellbeing, and support follow-through on treatment plans and referrals. • An Associate's degree and two years experience and/or a bachelor's degree
• Knowledge on how to provide holistic and therapeutic case management services.
• Experience working in the mental health field.
• Computer and documentation skills.
• Ability to solve problems and deal with complex situations and stay calm.
