Community Care Network is looking for Community Direct Support Professionals to join our team!
Community Care Network is comprised of Rutland Mental Health Services and Rutland Community Programs. Our mission is to enhance the well-being of our communities, individuals and families through responsive, innovative and collaborative human service
About the Role:
Opportunities are currently available for Direct Support Professionals to provide one-on-one support to adults with development disabilities in the community and in their homes throughout Rutland County. This position assists individuals in a variety of day-to-day tasks, such as but not limited to: behavioral needs, transportation, medical management, and more. This position will assist in planning and carrying out individuals programs as well as implementing goals identified by the Individualized Service Agreement (ISA). This job is rewarding and incredibly transformative.
Responsibilities:
Provides a stimulating and positive experience in the community based on the interests and preferences of the individual.
Interacts with team members, community and support services appropriately to ensure continuity of services.
Communicates appropriate information, issues and concerns in a timely manner to their Direct Supervisor.
Maintains confidentiality according to agency policy and state law.
Maintains flexibility and is responsive to changes in daily routine.
Accommodates physical needs of individuals.
Document required paperwork (i.e. daily data, service logs, timesheets, mileage sheet…etc.) in a timely manner.
Provides transportation to individuals as necessary.
Participates in meetings, trainings, and supervision as scheduled.
Whats Required:
High School diploma or GED.
Previous experience working with individuals with developmental disabilities preferred.
Able to communicate directly, respectfully, and clearly.
Light to heavy physical requirements include: prolonged sitting, standing, walking, hiking in the natural elements, running, bending, physical support of others, lifting of items, and weight bearing in addition to sitting/standing in front of a computer.
Must have a valid drivers license, reliable transportation with personal automobile liability insurance coverage at or above the levels mandated by the State of Vermont.
*Successful completion of a criminal history background check after hire.
