HopeHealth, one of the largest independent nonprofit hospice and palliative care organizations in New England, is a national leader in serious illness and end-of-life care. Its academic affiliation with Brown University's Alpert Medical School as the major teaching affiliate for hospice and palliative care is the only affiliation of its kind in the United States. Founded in 1976, HopeHealth is the 2nd oldest hospice in the country and is celebrating its 50th anniversary in 2026 with a fundraising campaign to ensure that more people can live well with serious illness, receive hospice care when the time comes, and feel supported every step of the way.
HopeHealth employs over 500 medical, clinical, operations and administrative staff, with headquarters in Providence and offices in Wakefield, RI and Attleboro, MA. The HopeHealth Hulitar Hospice Center, a 30-bed inpatient unit in Providence, is the only freestanding hospice center in the region. Through its strong partnerships with regional health care organizations, HopeHealth serves more than 13,000 patients, and 2,000 children and adults in grief support programs. These services cover the entire state of Rhode Island-including Block Island-as well as 60 towns in southeastern Massachusetts.
This is a full time 40 hours per week position working 8:00am-4:30pm
About the Position:
HopeHealth is seeking a passionate, community-minded, bilingual Community Engagement Manager to lead initiatives that expand access to our services in the richly diverse communities across Rhode Island and Massachusetts. As the public-facing leader of this initiative, the Community Engagement Manager will collaborate with community organizations, religious and cultural leaders, and healthcare providers to educate, advocate, and serve.
The Community Engagement Manager will create and lead a Community Engagement Program that increases access to HopeHealth services. This role is key to advancing health equity, building trust, and fostering strong relationships with historically underserved groups. The ideal candidate is an experienced communicator in English and Spanish, skilled in cultural competence and cultural humility, and dedicated to helping individuals and families understand and access the care they deserve.
The Community Engagement Manager will report to the Senior Director, Community Relations and Clinical Outreach. They will collaborate with internal HopeHealth teams in all areas, including community relations, clinical outreach, community education, clinical services, marketing and communications, IT, data and evaluation, finance, and human resources to expand, develop, implement, and evaluate HopeHealth's community engagement programs and strategies.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Community Partnership Development
Serve as a trusted bridge between HopeHealth and community stakeholders.
Build strong, lasting partnerships with:
Community health centers
Faith-based institutions
Cultural and social service organizations
Local nonprofits and advocacy groups
Recruit and manage a Community Advisory Council of diverse leaders and healthcare professionals and facilitate regular meetings to incorporate Council feedback into strategic program planning.
Community Education & Awareness
Facilitate the expansion of engagement, training and education to community healthcare providers, social service agencies, and members of the community.
Plan and lead culturally relevant workshops, community conversations, and awareness campaigns about hospice, palliative care, and serious illness support.
Extend HopeHealth's participation at community events, health fairs, and cultural gatherings.
Create and supervise a team of Community Health Workers, trusted public health liaisons linking communities to care and support.
Access & Navigation Support
Help individuals and families navigate access to care and reduce systemic barriers.
Offer empathetic guidance to those unfamiliar with hospice or palliative care systems.
Internal Cultural Competency & Advocacy
Collaborate with Human Resources and Clinical Leadership to develop and deliver training on culturally responsive care practices.
Advise marketing/communications teams on inclusive language, imagery, and message design.
Advocate internally to ensure patient care is linguistically and culturally aligned with community needs.
Data, Feedback & Continuous Improvement
Design and implement methods to collect community feedback, identify service gaps, and measure engagement outcomes.
Present findings and recommendations to HopeHealth leadership to inform strategy and improve accessibility.
