The Tribe is governed by a nine-member Tribal Council who serve staggered terms. The LTBB Government presently employs over 200 employees across 3 branches and more than 20 departments, ranging from Healthcare to Natural Resources.

Duties: ESSENTIAL DUTIES AND RESPONSIBILITIES: Provide excellent customer service to clients and vendors and fellow employees, and represent LTBB Health Department in a professional, positive manner. Recognizes and is sensitive to the cultural dimension inherent in the practice of health education in Native American communities. Must be culturally competent and meet the health literacy and linguistic needs of target populations. Learn and maintain knowledge of program services available to Tribal Citizens through LTBB Government programs and outside agencies. In conjunction with Health leadership, develop referral program from various programs to the Case Manager. Meet with clients and their support networks, including family and friends, to identify barriers and client needs. Develop appropriate service plans with clients with timelines and goals to ensure implementation of appropriate services based upon client biological, psychological and social factors. Coordinate tasks with Community Health Worker to fulfill needs of clients. Maintain ongoing oversight of client needs making appropriate referrals to LTBB Government departments including Health, Elders, Human Services, Housing, Education, Law Enforcement, and others. Make referrals to outside agencies when necessary. Assist with scheduling appointments for various services and completing applications for various programs as needed. Review scheduled appointments with client contact to improve attendance rate. Follow up directly with client for missed appointments. Provide transportation to appointments as a last resort. Maintain accurate and confidential client progress notes including referrals, visits, and other notable interactions. Provide on-going supportive and/or case management functions in accordance with identified health and wellness needs, and the strategies within the service and/or treatment plan in order to assist the clients achieve the stated goals and objectives. Participation in regular task force meetings to identify what is working and what needs to be adjusted to meet the ongoing needs of clients. Participates in inter-department planning and service coordination as required to improve and enhance service continuity and effectiveness for clients. Participates in weekly department staff meetings, other applicable staff meetings and case review. Maintain collaborative relationships and communication with LTBB Government departments and community...SEE FULL DESCRIPTION ON WEBSITE Qualifications: KNOWLEDGE, SKILLS, AND ABILITIES (KSAs): Must be knowledgeable of Native culture and traditions or willing to learn. Must be a self-starter with excellent organizational and time management skills to effectively coordinate multiple projects and duties with absolute attention to detail. Must be reliable and dependable. Must understand grant funding and programs. Must have knowledge of prevention, early intervention, treatment activities and resources. Must have advanced verbal and written communication skills and the ability to present effectively to various size groups. Must be able to effectively use computer software programs including Microsoft Excel, Word, and Outlook. Must have excellent work history, including with LTBB, if ever employed by LTBB. Hours may vary, must be able to work some weekend and evening hours; travel may be required. Must be able to accept constructive feedback. Must be able to work as part of a team in delivery of Health Department goals. EDUCATION AND EXPERIENCE: Required, in addition to KSAs: Bachelor''s degree from an accredited college or university in social work and at least one year of full time work experience working with at-risk individuals, families, and elders. Prefer experience working in Tribal community. COMMENTS: Indian preference will apply (Preference will be given to qualified Native American applicants in accordance with the LTBB Indian Preference Statute). Successful applicant must pass extensive background investigation and fingerprint screening. Must possess valid driver''s license, be insurable, and have reliable transportation. Individual must have a positive job history with former employers, including LTBB. Individual must take TB test and get influenza vaccination annually due to working in a health setting.
