THE POSITION

Are you motivated to use your nursing expertise to help improve the health of your community? As a Community Health Nurse with the Department of Health, you will have the opportunity to apply your skills in public health outreach, disease prevention, and patient education. You collaborate with partners, support clinical programs, and help address key health priorities in the region. This role provides a rewarding chance to support wellness and build healthier communities throughout Pennsylvania.

DESCRIPTION OF WORK

This position supports public health efforts by evaluating local health needs, providing clinical services, and assisting communities with education and outreach initiatives. It plays an important role in helping the Department of Health strengthen disease prevention and promote wellness throughout the region. As a Community Health Nurse, you will perform the following duties:

  • Health Assessment: Analyze local health data to identify needs and guide public health initiatives
  • Community Outreach: Link program resources with partners and strengthen community based health efforts
  • Program Monitoring: Track communicable disease programs by accessing public health information systems
  • Education Delivery: Provide health education on various public health topics for individuals and groups
  • Clinical Services: Offer immunizations, case management, and disease prevention services following public health protocols
  • Disease Investigation: Conduct surveillance and investigations to prevent and control outbreaks

Work Schedule and Additional Information:

  • Full-time employment, 37.5 hours per week
  • Work hours are 8:00 AM to 4:00 PM, Monday - Friday, with a 30-minute lunch.
  • Work infrequently requires statewide travel and irregular or flexible work hours, including overnights, weekends, and holidays, in accordance with policy.
  • Telework: You will not have the option to telework in this position.
  • Salary: In some cases, the starting salary may be non-negotiable.

REQUIRED EXPERIENCE, TRAINING & ELIGIBILITY

QUALIFICATIONS

Special Requirements:

  • This position requires active authorization to practice as a Registered Nurse in Pennsylvania.
  • All employees possessing an active temporary practice permit must obtain licensure as a Registered Nurse within the one (1) year period defined by the Pennsylvania State Board of Nursing

  • You must be able to perform essential job functions.

Legal Requirements:

  • You must pass a background investigation.
  • This position falls under the provisions of the Child Protective Services Law.
  • Under the Law, a conditional offer of employment will require submission and approval of satisfactory criminal history reports including, but not limited to, PA State Police clearance, PA Child Abuse history clearance, and FBI Fingerprint clearance.
