    POSITION SUMMARY: Under general direction of the Chief Medical Officer, the Community Health and Outreach Director provides creative leadership to coordinate and execute community health activities and initiatives, develops agency and community partnerships, creates resources for community health improvement projects, and performs related work as required. This position is responsible for working with the community to coordinate community health planning and/or development activities to improve the health of Native Americans in the NVIH service area.

    ESSENTIAL JOB FUNCTIONS: The list that follows is not intended as a comprehensive list, but rather to provide a representative summary of the major duties and responsibilities. Incumbent(s) will be required to perform a variety of duties listed, including but not limited to:

    1. Plan, organize, direct and evaluate the efficacy of the Community Health and Outreach Department. Provide oversight, leadership and supervision for community health programs and outreach events. Ensure that programs developed are culturally sensitive and relevant to the Native American populations served by NVIH and/or comply with grant requirements.
    2. Provide effective leadership to support CH/OR staff in providing care management services to patients with complex health care needs; ensuring a patient-centered self-management plan that integrates access to medical, dental, and behavioral health services and lifestyle interventions to improve health outcomes.
    3. Contribute to the agency’s effort in identifying top priority health issues for NVIH patients by coordinating all phases of community health planning: assessment, planning, implementation and evaluation; and applying interpersonal skills to enhance collaboration between NVIH staff, community members and partnering agencies towards improving community health outcomes.
    4. Participate in administrative services including fiscal, contractual and general business activities including the preparation and monitoring of the Community Health and Outreach budgets. In collaboration with the CMO, CEO and CFO, discuss and set standards for the annual department budgets.
    5. Facilitate and participate in program quality improvement projects, as well as Government Performance Results Act (GPRA)/HEDIS/QIP activities to improve program workflows and patient health outcomes.
    6. Ensure community health education and promotion programs are developed to reduce chronic conditions with a focus on chronic conditions that impact Native American communities.
    7. Prepare effective grant applications, funding proposals and budget submissions to promote the development of resources to support innovative population-based health promotion efforts.
    8. Work in collaboration with department Directors to develop organizational goals that promote NVIH’s mission, policies and procedures.
    9. Develop or update job descriptions, onboarding checklists and continued trainings that provide staff with opportunities to grow in skillsets.
    10. Demonstrate effective team member orientation; actively promoting the mission of the Agency in interactions with community agencies, proposed funding sources, the public and fellow Agency staff.
    11. Other duties as assigned.

    Required Experience:

    REQUIRED QUALIFICATIONS: The following knowledge, skills, and abilities are required for an employee to successfully perform their duties.

    1. Bachelors degree required; preferably in Nursing, Healthcare Administration, Public Health or similar field.
    2. Five (5) years of experience in public health, public administration, health administration, or business administration, or a directly related field, which includes three (3) years of leadership experience with health-related program development and implementation.
    3. Valid California Registered Nursing License required, PHN certificate preferred
    4. Ability to successfully engage in community outreach, organization or coalition building.
    5. Leadership skills with the ability to motivate, inspire, coach, counsel and ensure team effectiveness; previous supervisory experience required.
    6. Experience working with various stakeholders towards successful development and implementation of projects and initiatives.
    7. Communicate orally and in writing in a clear, concise and effective manner and utilize word processing software and the Internet with a high level of competence. Experience writing, securing and managing grant activity (preferably with a health-based organization).
    8. Innovative and creative thinking in developing new approaches as needed, focus on results, exercise initiative, demonstrate ingenuity, think strategically, and show sound judgment in identifying and pursuing sources of program funding.
    9. Ability to engage in strategic actions to generate idea creation and execute agency-wide initiatives, navigate complex challenges, drive organizational growth and capitalize on emerging opportunities.
    10. Able to travel between sites and maintain a valid California driver’s license and insurability with the Agency auto policies along with maintaining reliable transportation.
    11. Versatility, flexibility, and a willingness to work within constantly changing priorities with enthusiasm as an innovative contributor of the senior leadership team.
