Position Summary: Under the direction of a licensed or registered nurse provides basic health and lifestyle assistance to assigned patients within their residences and within a given service area by performing the following duties personally. Responsible for ensuring that all critical tasks are fulfilled on a timely basis.

Critical Tasks:

Maintains exclusive focus upon patients, the quality of their care, their individual needs and their medical outcomes without significant distraction by administrative and non-patient contact activities.

  • Complies fully with all patient medical records and information privacy regulations, state and federal.

  • Takes all steps necessary to fully ensure patient/resident and medical data confidentiality and security by all clinic staff.

  • Ensures that patients and residents are comfortable and well cared for according to state and federal regulations as directed by supervising nurse.

  • Maintains awareness of comfort and safety needs of patients and residents.

  • Properly and safely handles and disposes of medical equipment including biohazardous materials.

  • Schedules some medical appointments, arranges for medical transportation, and delivers medications with 100% accuracy as prescribed, all under the direction of supervising nurse.

  • Fully complies with enterprise policies regarding all procedures and collection of specimens for laboratory analysis.

  • Provides diet education and guidance regarding self-care subjects to patients.

  • Properly and accurately documents all patient data and services provided on a very timely basis.

  • May provide limited personal services such as making beds, feeding patients, and ensuring residence environmental safety.

  • Observes, collects and records a variety of routine patient data.

  • Remains compliant with all enterprise medical care protocols.

  • Reports observations to supervisor and other appropriate staff members as appropriate while taking all steps to ensure proper service to patients.

  • Performs all duties only within competency. Does not perform services requiring licensure or certification.

  • Participates in health awareness and wellness activities within the assigned service area.

  • Demonstrates ability to regularly change priorities to accomplish all tasks despite frequent interruptions.

  • Demonstrates ability to clearly communicate, both orally and in writing, while performing all essential functions.

  • Performs other special projects and duties as assigned.

Required Qualifications:

  • High school graduate and diploma.
  • Current and continuing CPR certification.
  • Certified Nursing Assistant or Medical Assistant certification required.
  • Arizona driver's license in good standing.
  • Minimum of one year of patient facing experience, preferably public health/community health experience.
  • Demonstrated ability to relate to diverse cultures and specifically the Gila River Community and/or other Native American cultures.
