Community Health RN

Location: Community-Based (Service Area: Lake County Illinois) Job Type: Full-time Schedule: 8:00 AM – 4:30 PM (M-F, Tues-Sat, or Sun-Thurs available)

The Role

We are looking for a compassionate Psychiatric Registered Nurse to join a community based organization. This is a unique hybrid role blending psychiatric nursing, care coordination, and community-based case management.

Approximately 75% of care is delivered in client homes and community settings rather than a traditional clinic. You will be the primary link for individuals with serious mental illness, helping them maintain stability and move forward in their recovery.

Key Responsibilities

• In-Home Assessments: Conduct health and mental status assessments in the community or via telehealth.
• Medication Management: Provide education, monitor adherence, and track side effects.
• Care Coordination: Collaborate with an interdisciplinary team, including psychiatry and primary care providers.
• Crisis Intervention: Use trauma-informed clinical judgment to respond to crises; participate in a shared on-call rotation.
• Community Support: Transport clients in your personal vehicle to medical appointments and provide clinical guidance to LPNs.

Qualifications

• License: Active Illinois RN license (or eligible within 60 days).
• Education: Associate Degree in Nursing required (BSN or MSN preferred).
• Certifications: BLS or ACLS.
• Experience: 0–2 years of mental health experience; early-career nurses are welcome!
• Requirements: Valid IL driver’s license, reliable personal vehicle, and auto insurance (100k/300k).
• Mileage Reimbursement: 72.5 cents per mile
