Community Health Support Member

. AmeriCorps members are part-time to full-time members who serve to support nonprofit, faith-based, government, and community organizations across the country. Based out of the University of the South, the South Cumberland Plateau AmeriCorps operates in Coffee, Franklin, Grundy, Marion, and Sequatchie counties. The SCP AmeriCorps pairs civic-minded individuals with local organizations to tackle hunger relief, financial literacy, education and tutoring, substance abuse, health, housing access, and green space development. Service members receive a living stipend during their service and are eligible for the Segal Education Award at the end of their volunteer term. This award enables members to pay off qualified student loans or other qualified education expenses. Member Duties : The Community Health Support Member (CHSM) will focus on working in the community to address problematic components of the Healthcare system. This Member will collaborate with medical providers, primary care teams, and other patient support agencies to best improve and cater to local community needs. This role will assist partners in providing health-related services, including facilitating information on local social services and medical facilities. This CHSM will help patients to schedule appointments, utilize available resources, and assist in completing applications for relevant programs. This role will require the ability to plan, implement, and evaluate individual client care plans. The CHSM will document relevant activities, service plans, and medical results in an effective and confidential manner. In this role, the CHSM could be asked to develop community outreach programming to promote healthy habits, vaccination, or other similar healthy living initiatives.
