The Opportunity

FuntoNetwork is seeking a Community Health Team Tracking Tools Implementation Lead for an active RFP opportunity: Vermont DVHA NOA 03410-240-26 - Rural Health Data and Analytics Infrastructure Grant.

DVHA is offering a Rural Health Transformation Fund grant for eCQM calculation tools, community health team tracking tools, and primary-care data analytics infrastructure.

Core Responsibilities

• Coordinate implementation and adoption of community health team tracking tools, documentation, training, and stakeholder readiness.
• Review the RFP scope, source posting, supporting documents, deadline, and client requirements.
• Coordinate with FuntoNetwork and related consultants to support a complete, professional response.
• Prepare documentation, analysis, recommendations, deliverables, or technical inputs related to this role.
• Communicate clearly with technical and non-technical stakeholders as needed.
• Support quality control, status updates, and final submission readiness.
• Define the Community Health Team Tracking Tools Implementation Lead work package: Coordinate implementation and adoption of community health team tracking tools, documentation, training, and stakeholder readiness.
• Gather role-specific inputs: Collect the source materials, stakeholder decisions, system details, and operational constraints needed from State of Vermont Department of Vermont Health Access.
• Perform the specialist analysis: Apply the methods, standards, tools, and professional judgment required for this role's assigned scope.
• Develop the assigned deliverables: Produce the role-specific designs, recommendations, configurations, records, or operational outputs required by the project.
• Validate the specialist work: Check the deliverables against source requirements, professional standards, identified risks, and acceptance criteria.
• Complete role handover: Resolve review comments and deliver final files, decisions, open items, operating guidance, and supporting evidence.

What You Bring

• Relevant experience for this RFP-based role.
• Strong written communication, documentation, and stakeholder coordination skills.
• Ability to work against proposal deadlines and client requirements.
• Experience supporting public-sector, enterprise, or consulting projects is preferred.
• Attention to detail and ability to follow structured submission requirements.

Earnings, Payments, and Tax Records

Consultants will be able to track project earnings, payment status, and tax records inside the FuntoNetwork mobile app.

Approved payments for this RFP opportunity will be processed through the app, and consultants will be able to request withdrawals from their available balance directly inside the app.

The app will also provide access to relevant payment history and tax-related records for consultant tracking and reporting.
