    Description

    Community Health Workers (CHWs) assess and address clients'' social needs by developing actionable plans to connect individuals with resources that address Social Drivers of Health (SDOH). CHWs hold an essential role in advancing community health by addressing systemic barriers and empowering individuals to improve their quality of life.

    Under indirect supervision, CHWs coordinate with medical providers, primary care teams, social services, and community organizations to deliver short-term care coordination, educational support, and resource navigation. CHWs may also provide peer support in areas such as mental health, substance use recovery, housing instability, food insecurity, and other social encounters.

    This role offers comprehensive training to obtain CHW certification, supporting skill development and professional growth. CHWs provide direct support in clinical, community, and home settings, focusing on improving clients'' overall health and well-being and their access to services.
