As a member of the Patient Centered Medical Home team, the Community Health Worker will assume responsibilities for the care coordination of patients. The CHW works collaboratively with internal and external entities in order to remove barriers to care and ensure smooth care transitions for patients/families. The CHW works with patients to address their needs by the serving as the liaison between the patient and the rest of the care team. The CHW will identify and help to close care gaps, access community services, provide support and education to the patient and family/caregivers, and engage the patient in their healthcare. Ability to communicate effectively, build good relationships, show compassion and empathy and exercise good judgment. Familiarity of community based resources, ability to work as part of a team, self-driven and motivated. Familiarity with healthcare systems and settings. High School diploma required, Associates Degree preferred. Experience as a Medical Office Coordinator or Medical Assistant in an ambulatory setting preferred. Proficiency with Microsoft Office, general knowledge of electronic medical records preferred.
