Job Summary
The Community Health Worker supports a CDC-funded Community Diabetes Initiative in partnership with HealthCare Access Maryland and the Baltimore City Health Department. This role conducts outreach, screening, and referrals to diabetes prevention programs and essential resources, addresses social determinants of health, and connects Baltimore City residents with education and care to prevent or manage type 2 diabetes.

Duties and Responsibilities
The primary responsibilities of this position are:


Administration
  • Attend and participate in partner meetings with community agencies/organizations
  • Consistently achieve productivity goal and performance measures established for the Community Health Worker  
Case Management
  • Document assessment, contacts, and services in electronic medical record 
  • Enroll uninsured clients into available health insurance programs; assist them in choosing a Managed Care Organization and identifying a primary medical provider.
  • Maintain ongoing communication and follow-up with clients for up to 12 months while they participate in the Diabetes Prevention Program)
  • Provide client centered services in a trauma informed approach.
  • Effectively use motivational interviewing techniques to engage and coordinate care/services to clients.
  • Provide education and system navigation of the Maryland Medicaid insurance benefit package.
  • Refer Baltimore residents into Diabetes Prevention Programs, Social Needs resources.
  • Conduct social determinants of health screenings and health risk assessments.
Outreach & Education
  • Perform client outreach, inclusive of neighborhood canvassing, home visits, and community events.
  • Provide diabetes education, disseminate educational materials, and increase awareness of diabetes prevention.
Human Resources
  • Accurately and timely submit timesheets.
  • Attend all required trainings.

QUALIFICATIONS:
Education, Experience, Certifications and Licensures:
  • High School Diploma or GED with significant human services experience. Bachelor’s degree in a Human Services field is preferred.
  • Reliable transportation is required for community outreach, home visits, canvassing, and attendance at community events.
  • 1 Year experience working in a Community Health Worker (CHW) role and/or direct experience providing outreach and client services in Baltimore City in the areas of Medicaid System Navigation, Social Needs Assessments and Care Coordination
  • Community Health Worker Certification preferred but not required
  • Bilingual candidates are strongly preferred. (Spanish)

KNOWLEDGE, SKILLS & ABILITIES
  • Strong computer literacy skills, specifically with MS Word, Excel, Outlook and SharePoint (365).
  • Organizational skills with the ability to meet a demanding workload
  • Detail oriented to complete requirements of files and contract compliance.
  • Sensitivity to cultural and socioeconomic characteristics of population served.
  • We practice the “Housing First” philosophy. Knowledge is preferred; support is required.

Compensation 
In compliance with Maryland's wage transparency requirements, the compensation for this role has been included in the posting.
