Job Summary
This is a hybrid position. 3 days on site and 2 days remote. This can change at any given time due to department needs/demands.
- Complete Health Risk Assessments along with diabetes, hypertension, and depression assessments monthly.
- Assist with community resources and social determinates of health.
- Enroll eligible members in disease management programs and provide disease specific & preventive care according to program requirements.
- Support the treatment plan prescribed by PCPs.
- Must maintain a minimum case load of 100-150 patients within 90 days of onboarding.
- Assist patients in scheduling appointments.
- Accurately documenting in MHN portal for each patient.
- Follow-up with members who have recently been in the ER and schedule 7-day follow-up appointments with PCP.
- Follow-up with members who have either admitted as an observation stay or inpatient stay and schedule 7-day follow-up appointments with PCP.
- Must be proficient in Excel.
- Must have excellent computer skills.
MINIMUM WORK EXPERIENCE
1 year of health education experience preferred.
WHAT YOU WILL NEED
- High school degree or GED
- Drivers license preferred
- Basic computer knowledge (Microsoft Word, Excel)
- Hours are flexible but will include some evening and weekend hours scheduled in advance
- Ability to assist with health education
- Ability to talk to small groups to provide health education
- Ability to provide case management under supervision
- Knowledge of Chicagos Westside and/or Southside communities; residents of Westside or Southside communities preferred
- Some positions may require Spanish
