As a Community Health Worker (CHW), you will serve as a trusted advocate and connector, empowering individuals and families to access behavioral health services, community resources, and support that improves overall well-being. Through outreach, education, and relationship-building, you will help reduce barriers to care and strengthen healthier communities.
Job Description
- Conduct community outreach, presentations, and networking activities to engage individuals with a history of trauma, substance use, mental health concerns, or other behavioral health risk factors while promoting awareness of available program services.
- Build and maintain partnerships with community organizations and social service agencies by developing Memoranda of Understanding (MOUs), coordinating referrals, and identifying barriers to accessing care.
- Screen individuals identified through outreach or referrals using approved assessment tools, including Adverse Childhood Experiences (ACEs) and Social Determinants of Health (SDOH), to determine service and support needs.
- Connect participants to behavioral health, healthcare, and community resources by providing referrals, care navigation, follow-up, and appropriate interventions based on individual needs.
- Maintain accurate documentation, support implementation of evidence-based program curricula, and ensure compliance with organizational policies and grant requirements.
Requirements
- High School Diploma, GED, or experience equivalency.
- At least 2 years of experience working in social services, networking with social service agencies, or working with at risk populations in the community or schools.
- Current Texas driver's license, reliable transportation, and liability insurance are required.
- Bilingual in English and Spanish preferred.
- Current Texas Community Health Worker (CHW) Certification or ability to obtain certification within 6 months of hire.
