Community Health Worker - Contractor

Position Summary

The Community Health Worker (CHW) - Contractor is a trusted frontline member of the FQHC care team, responsible for community outreach, point-of-care testing (A1c, glucose, blood pressure), scheduling support, and connecting patients to social determinants of health (SDOH) resources. The CHW works directly with Southside Chicago residents to close care gaps, support chronic disease management, and improve patient engagement.

Responsibilities

• Conduct community outreach in homes, shelters, churches, schools, and public spaces.
• Perform point-of-care screenings: A1c, blood glucose, blood pressure, BMI.
• Document screenings and outreach activities in the electronic health record (EHR).
• Facilitate appointment scheduling and warm handoffs to clinical teams.
• Complete SDOH screenings and connect patients to internal and external resources.
• Support care transitions and follow-up needs for high‑risk patients.
• Participate in team huddles, case conferences, and quality‑improvement activities.

Qualifications

Required:

• High school diploma or equivalent.
• Familiarity with Southside Chicago neighborhoods and cultural dynamics.
• 1+ year of community outreach, patient engagement, or care‑coordination experience.
• Ability to conduct health screenings and follow basic clinical protocols.
• Strong communication skills and cultural humility.
• Reliable transportation for community fieldwork.

Preferred:

• Certified Community Health Worker (CCHW) or willingness to obtain within 6-12 months.
• Experience working with Medicaid populations and community‑based organizations.
• Bilingual (English/Spanish) highly preferred.

Work Environment

• Field-based community outreach (60-70%) and clinic-based work (30-40%).
• May require walking, lifting up to 20 lbs., and working outdoors.
