The Community Health Worker Coordinator is responsible for leading, coordinating, and supporting Community Health Worker (CHW) programs and initiatives that improve access to healthcare services, promote health equity, and address the social determinants of health within the communities served. This position oversees the daily operations of the Community Health Worker team, training, and professional development, and ensures the effective delivery of outreach, education, care coordination, and patient navigation services.
- Serve as the primary point of contact for Community Health Workers (CHWs) regarding day-to-day operational questions, case support, workflow guidance, and problem resolution.
- Provide onboarding and training for newly hired CHWs, including orientation to organizational policies, documentation standards, community resources, workflows, and best practices.
- Provide operational coverage during CHW absences by managing urgent patient needs, completing emergency department follow-up calls, monitoring team inboxes for time-sensitive tasks, and ensuring continuity of care through appropriate patient outreach and follow-up.
- Develop and facilitate ongoing staff education, coaching, and skill-building opportunities to support professional growth and service quality.
- Develop and maintain knowledge of billing requirements, documentation standards, and applicable regulations to support program compliance; collaborate with billing departments to provide staff training, monitor billing accuracy, and track ongoing billing compliance across Community Health Worker (CHW) services.
- Lead monthly Community Health Worker (CHW) team meetings, including developing and distributing meeting agendas, facilitating discussions, communicating program updates, and documenting and distributing meeting minutes and action items.
- Monitor CHW productivity metrics, caseloads, outreach activities, and program goals to ensure performance expectations and service standards and report productivity to PHPM.
- Conduct regular check-ins with CHWs to review workloads, identify barriers to success, and provide guidance, mentorship, and performance support.
- Assist in creating schedules, coverage plans, and task assignments to maintain efficient program operations and continuity of services.
- Review documentation and reporting for accuracy, completeness, and compliance with organizational requirements.
- Collaborate with leadership to implement program initiatives, workflow improvements, and quality improvement strategies.
- Support CHWs in navigating complex client situations by connecting staff with appropriate internal departments, supervisors, or community resources.
- Maintain current knowledge of community services, healthcare resources, social service programs, and referral networks to support both staff and clients.
- Participate in hiring activities, including interviewing candidates, providing onboarding support, and assisting with staff evaluations as requested.
- Promote a positive, team-oriented work environment that encourages collaboration, accountability, communication, and employee engagement.
- Track and analyze program data, outreach outcomes, and operational trends to identify opportunities for improvement and support reporting needs.
- Ensure CHWs adhere to confidentiality standards, organizational policies, safety procedures, and ethical guidelines.
- Track Continuing Education Unit (CEU) requirements and certification renewal deadlines for Community Health Workers (CHWs) to ensure ongoing compliance with organizational, state, and program standards.
- Act as a liaison between CHWs and PHPM by communicating staff concerns, operational challenges, and recommendations for program enhancement.
- Assist with community outreach initiatives, health education events, and partnership-building activities as needed.
- Assist patients with identifying, applying for, and maintaining appropriate healthcare coverage programs, including Medicaid, Medicare, and marketplace plans.
- For those not already attending, assure attendance in HSCC meetings for their counties.
- Help develop standardized procedures, documentation, training materials, and EHR workflow guides to improve consistency across the CHW team.
- Perform additional administrative, operational, and program support duties as assigned to support organizational goals and community health initiatives.
- Continuously expands knowledge and understanding of community resources, services and programs provided.
Required Training & Experience:
- Use of Electronic Health Records
- Word & Excel
- Possesses a current State of Michigan driver's license and valid automobile insurance.
- Possesses a current CHW certification, if not, will be required to obtain one.
