JOB SUMMARY Under the supervision of a higher classified employee, the Community Health Worker (CHW) will perform a variety of outreach and customer service activities in both an office and non-office setting. The CHW role exists to build strong, mutually-beneficial relationships with community members. These connections can increase access to available health services and can support changes to programs and services where needed to better support community health for all. Additionally, the CHW may provide support to all divisions within the Health Department, such as acting as a Health Department representative in a variety of community settings. This position will support the Childrens Special Health Care Program. DEPARTMENT-WIDE CORE COMPETENCIES

DESIRED QUALIFICATIONS Proficiency in a second language. Community Health Worker Certification recognized by the State of Michigan. Strong community connections and/or similar life experiences to the community or population served. Experience working with people. Experience working with children with special needs is desirable. Parent of a special needs child is desirable. LICENSES AND CERTIFICATIONS Possession of a valid Michigan vehicle operators license. Availability of automobile, van, or truck. PHYSICAL DEMANDS Duties require sufficient mobility to work in a both a normal office setting and in the community. Use of standard office equipment including as well as specialized devices to communicate with hearing and speech impaired clients. Requires ability to read printed material and must be hearing and speech sufficient to communicate in person or over the phone. Requires ability to drive to multiple work locations. These requirements may be accommodated for otherwise qualified individuals requiring and requesting such accommodations. EDUCATION/EXPERIENCE Possession of a high school diploma or equivalent. This is an entry-level classification; no specific prior experience is required.
