Position Overview:

Under the direct supervision of the Outreach Cohort Supervisor, the Community Health Worker is responsible for supporting Harris County Public Health's outreach activities and serves as the key liaison to educate, promote, and connect community members to all HCPH programs. This position will primarily work in the community through various outreach activities within Harris County, while also completing in-office tasks/projects to support overall outreach team objectives.

Duties and Responsibilities:

  • Responsible for diverse outreach, education and community coordination activities such as canvassing local businesses/organizations, attending community events, conducting outreach phone calls, and giving presentations.

  • Serves as a subject-matter expert to meet outreach objectives for HCPH programs and recommends changes to improve outreach strategies and support the creation/updating of HCPH program trainings for the entire outreach team.

  • Maintains knowledge/skills to educate, promote, and connect community members to all HCPH programs/services.

  • Tracks, collects, and submits outreach service data on a daily and weekly basis.

  • Builds and maintains relationships with stakeholders and organizations to create and sustain innovative partnerships.

  • Responsible for initiating referrals to HCPH programs/services, while ensuring community members confidentiality.

  • Create and deliver educational presentations/workshops on public health topics for diverse target audiences and other duties as assigned, including special tasks involved with responding to emergency events.

Requirements

Education:

  • High School diploma.

Knowledge, Skills and Abilities:

  • Intermediate proficiency in Microsoft Office.

Certifications:

  • Community Health Worker Certification (CHW).

General Information

Position Type, and Typical Hours of Work:

  • This is a full-time, regular position.

  • Days and hours of work are typically Monday through Friday, from 8:00 am to 5:00 pm.

  • Other days including weekends, and evening hours as necessary for Outreach needs and to attend community events.

  • This position requires the ability to travel within Harris County jurisdiction.

Work Environment:

  • This job operates in both field and office environment. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines.

Physical Demands:

  • This is a largely sedentary role; however, some duties would require the ability to lift up to 20 pounds and bending & standing for periods at a time.
