Job Announcement

The Fairfax County Health Department is a progressive public health leader serving 1.2 million residents and providing services that promote population health, protect public health and the environment, and ensure residents equitable access to health services and information.

Under the direction of the Public Health Nurse (PHN), this position works directly with clients enrolled in the Chronic Disease Care Management program to support the client in effective management of his/her chronic health conditions, self-care, linking with community resources and adopting healthy behaviors. Establishes trusting relationships with clients/caregivers and families while providing general support and encouragement. Provides these services in the client's home, community and clinic settings. Collaborates with the health care team, providers and community partners in implementation of the client's individualized health care plan. Provides social support, informal counseling, and advocates for individuals and community health needs, and provides services such as home visits, small group education and community outreach.

Note: Human Services Assistant position work schedules are on-site and typically Monday through Friday and occasional evenings, as needed.

Required Knowledge Skills and Abilities

  • Knowledge of the basic principles of human behavior;

Employment Standards

MINIMUM QUALIFICATIONS:

Graduation from high school or G.E.D issued by state department of education; plus three years of work experience, including two years in some area of human services (related two-year [associate] degree may be substituted for the required human service experience).

CERTIFICATES AND LICENSES REQUIRED:

  • Valid drivers license.
  • Cardiopulmonary Resuscitation (CPR) certification (Required within 60 days).
  • Automated External Defibrillator (AED) certification (Required within 60 days).
  • Successful completion of Language Proficiency Testing upon hire.
  • Successful completion of Community Health Worker formal training program upon hire.

NECESSARY SPECIAL REQUIREMENTS:

The appointee to this position will be required to complete a criminal background check, Child Protective Services Registry check, driving record check, and tuberculosis (TB) test to the satisfaction of the employer.

Candidate must pass the following upon hire: Successful completion of Language Proficiency Testing; Successful completion of Community Health Worker formal training program.

This position is emergency/essential services personnel. (Required to report for duty during inclement weather and/or other emergencies.)

Additional Work Schedule Requirements: Flexible dependent on department needs. May occasionally start as early as 7:00 a.m. or work as late as 9:30 p.m. based on service demands and occasionally on weekends. May be assigned to work at various sites. Must provide own transportation.

PREFERRED QUALIFICATIONS:

  • Bilingual or multilingual in English and another language, with exceptional verbal and written communication skills (must pass language proficiency exam).
  • Experience in Community Health.
  • Experience conducting a social or health needs assessment to determine services needed for a client.
  • Experience working with diverse multicultural populations.
  • Ability to work well with multi-disciplinary team members and community partners.
  • Knowledge and experience with care-coordination and/or health coaching.
  • Ability to reach out and maintain positive community relationships.
  • Ability to exercise tact, discretion, initiative, and independent judgment.
  • Excellent customer service skills with the ability to interact in a friendly, service-oriented and professional manner.

PHYSICAL REQUIREMENTS:

Job is generally sedentary in nature, however, job entails walking, standing, sitting, climbing stairs, reaching and bending; may be required to lift up or carry up to 25 lbs. occasionally; uses hands to grasp, handle, or feel. Visual acuity is required to read data on a computer monitor; ability to operate keyboard driven equipment and computer. Position frequently communicates and must be able to exchange accurate information with others verbally and in writing. Generally, works in an office environment yet may occasionally be required to perform job duties outside of the typical office setting. Ability to drive a motor vehicle. All duties performed with or without reasonable accommodations.
