    Description

    A growing healthcare technology company is seeking a Community Health Worker to support patient engagement, care coordination, and enrollment efforts focused on improving cardiovascular care.

    Role Highlights

    • Hybrid position: 2–3 days remote / 2–3 days in the community
    • Travel to hospitals and healthcare facilities throughout Chicagoland
    • Engage patients in person, by phone, text, and email
    • Educate patients about available care programs and support enrollment
    • Build relationships with patients, caregivers, hospital teams, and clinical staff
    • Help coordinate discharge plans and successful transitions back home
    • Maintain patient documentation and activity within the company’s EHR
    • Collaborate with an interdisciplinary healthcare team
    • Meet monthly outreach, enrollment, and engagement goals

    Ideal Background

    • 3+ years of healthcare experience in a hospital, clinic, physician practice, or similar environment
    • Background as a Community Health Worker, Medical Assistant, LPN, CNA, or similar role
    • Cardiology experience preferred
    • Strong patient communication and relationship-building skills
    • Comfortable explaining healthcare programs and engaging patients
    • Tech-savvy and able to learn new systems quickly
    • Care coordination or interdisciplinary team experience preferred
    • Telehealth experience is a plus
    • Spanish-speaking is a plus
    • Valid driver's license, reliable transportation, and ability to travel locally
    • Available Monday–Friday, 8:00 AM–5:00 PM
