Summary

The Community Health Worker II is a frontline public health worker who is a trusted member of and/or has extensive understanding of the community served. This trusting relationship enables the worker to serve as a liaison/link/intermediary between health/social services and the community to facilitate access to services and improve quality and cultural competence of service delivery. The Community Health Worker II also builds individual and community capacity by increasing health knowledge and selfsufficiency through a range of activities such as outreach, community education, social support, and advocacy. This advanced position will provide leadership in their service area through activities to support the program's day to day functioning and efficiency.

Experience:

Required: * 5-7 years

Education: Preferred: * Bachelor's Degree

Licenses And Certification:

Required: * Certified Community Health Worker
