Community Health Worker

Position Overview

As a Community Health Worker, you'll work directly with Medicaid members, helping them navigate healthcare benefits, complete screenings, and connect with essential community resources. You'll serve as a trusted advocate, providing support and guidance while building meaningful relationships with individuals who may be facing complex social and healthcare challenges.

Responsibilities

  • Conduct outreach and build relationships with Medicaid members.
  • Complete member screenings and assess individual needs.
  • Assist members with navigating healthcare benefits and available community resources.
  • Coordinate referrals through the Unite Us platform.
  • Provide education, advocacy, and ongoing support to help members access appropriate services.
  • Handle sensitive and emotionally challenging conversations with empathy and professionalism.
  • Collaborate with internal teams and community organizations to improve member outcomes.
  • Participate in monthly community outreach events and engagement activities.

Qualifications

We're looking for candidates with:

  • Experience working for a nonprofit organization, community-based organization, or social services agency
  • Previous case management or care coordination experience
  • Knowledge of Medicaid, community resources, or social service programs
  • Excellent customer service and communication skills
  • Ability to build trust and rapport with diverse populations
  • Strong organizational skills with the ability to manage multiple cases and priorities
  • A compassionate, patient-centered approach to supporting vulnerable populations

Schedule & Compensation

  • Pay Rate: $24-26/hr depending on experience
  • Position Type: Temp to Hire
  • Schedule: Full-time (40 hours/week)
  • Location: Hybrid - Melville, NY 11747
  • Initial training conducted onsite
  • Transition to a hybrid work schedule after training
  • Monthly community outreach days required
